THE TOURNAMENT FEATURES:

12:30pm Shotgun Start / Scramble Format Golf Cart,
Range Balls & Box Lunch
Dinner and Awards
Prizes for the Longest Drive and Closest to the Hole

Collared shirts and no blue jeans.
Steel spikes are not allowed on the course.

DEADLINES:

The deadline for sponsorships is April 30, 2013.
The deadline for individual player entry is May 1, 2013.

$275 – Per golfer, includes box lunch, cart and dinner
$250 – Laguna Honda Hospital Employee
$60 – Awards Dinner only

SPONSORSHIP:

$1000 – One foursome package
$2500 – One foursome with printed materials and 2 tee signs
$5000 – SILVER One foursome package with banner, printed material, recognition plaque and two tee signs
$10,000 – GOLD Two foursome packages with banner, printed materials, an actual pin flag, two tee signs, a scorecard mention, and a recognition plaque
